If I have some packages mismatched, they are put on hold.
Can these packages ever be upgraded?

All mismatch tests except for the manually set ones that currently have none in them, are dynamic, and are based on the currently reported version numbers in apt itself.

But which package or package group is being held?

It could be a bug as well, please post a link to an uploaded /var/log/smxi.log from your last smxi run, use paste.debian.net and I'll let you know, I just fixed some issues with gnome package groups that had a bug in them that made them always hold some gnome packages, but that should now be fine.
